P61 COMBINED NON-PENETRATING FILTERING SURGERY AND CATARACT EXTRACTION IN TREATMENT OF ANGLE CLOSURE GLAUCOMA PATIENTS WITH CATARACT A RETROSPECTIVE REVIEW Dr Nancy SY Yuen, FHKAM, Dr Orlando CC Chan, MBBS, Dr SP Hui, FHKAM Department of Ophthalmology, Tung Wah Eastern Hospital, Hong Kong PURPOSE To review the result of non-penetrating filtering surgery combined with phacoemulsification done by a single surgeon in treatment of angle closure glaucoma patients with cataract in Chinese patients. METHODS 29 eyes of 26 patients who have cataract and angle closure glaucoma who had undergone combined nonpenetrating filtering surgery and phacoemulsification in year 2001 to early 2003 were reviewed. Visual acuity, intra-ocular pressure and complications were analyzed. RESULTS The study included 29 eyes in 26 patients, followed up for an average 17 months range 7.3 to 36 months ; . Visual acuity improvements were noted in 83% 24 in 29 eyes ; and remain the same after operation in 4 eyes. IOP was reduced from 20.28 3.86 mmHg mean SD ; preoperatively to 16.52 2.76 mmHg mean SD ; postoperatively at last follow up visit p 0.001 ; . The number of antiglaucoma medications was reduced from 2.86 0.79 mean SD ; preoperatively to 0.56 0.87 mean SD ; at last follow-up p 0.001 ; 16 of 29 eyes 55% ; achieved complete success with IOP 21 mmHg without antiglaucoma medications and 25 of 29 eyes 86% ; achieved qualified success with IOP 21mmHg with or without medications at last follow-up visit. There were four failures defined as uncontrolled IOP requiring further operation or oral drug treatment. Complications include one accidental anterior chamber puncture and one iris plug intra-operatively. Postoperative complications include 1 choroidal effusion, 3 wound leak requiring repair, 2 with punctate epithelial erosions. No shallow anterior chamber, hyphema, hypotony or infection was encountered. CONCLUSIONS Combined non-penetrating filtering surgery and phacoemulsification could be a safe and effective surgical options for management of angle closure glaucoma patients with cataract!
