Aims: Chronic zygapophyseal joint pain or discogenic pain are lacking effective treatments. Radiofrequency denervation RD ; is one of the promising therapeutic modalities. Methods: The objective was to assess the effectiveness of RD in the treatment of musculoskeletal pain disorders. Original articles for this systematic review were identified by electronically searching MEDLINE, PsycLIT, EMBASE, and the Cochrane Library to February 2002, hand screening references, and consulting experts in the field. Two reviewers selected the randomized controlled trials RCTs ; that met the inclusion criteria, extracted the data, and assessed the main results and methodological quality of the selected trials. Qualitative analysis was conducted to evaluate the level of scientific evidence. Results: Of seven relevant RCTs, six were considered to be high-quality. The selected trials included 275 randomized patients, 141 of whom received active treatment. One study examined cervical zygapophyseal joint pain; two cervicobrachial pain; three lumbar zygapophyseal joint pain; and one, discogenic low back pain. Sample sizes were small, follow-up times short, and there were deficiencies in patient selection, outcome assessments, and statistical analyses. Conclusions: There is limited evidence that RD offers shortterm relief for chronic neck pain of zygapophyseal joint origin and for chronic cervicobrachial pain, and conflicting evidence for its effectiveness for lumbar zygapophyseal joint pain. There is limited evidence suggesting that intradiscal radiofrequency may not be effective in relieving discogenic low-back pain. The most traditional application of ESI is the translaminal approach, where the needle is placed blindly in the epidural space. The success rate for blind conventional lumbal ESIs, using epiduralograms, has been rapported at 92% in patients with no prior back surgery 3 ; . The transforaminal approach has increased in popularity over recent years. The availability of fuoroscopy has made it possible, but it's role is still somewhat controversial. The belief is that placing the medication close to the patological process e.g. intervertebral disc herniation ; is more effectuous. Also, it will possibly decrease incidences of complications.
