Microbiol., 71, p214, 2005 ; . BHI liquid medium inoculated with both strain Niu-O16 and Julong 732 was incubated for 4 days with 1.2 mM of daidzein in an anaerobic chamber. The bacterial culture broth was sampled every 12 hr and analyzed. Metabolites in the broth were identified on the bases of EI-MS, NMR and UV spectral analysis on HPLC. Enantiomers were separated on chiral stationary-phase HPLC and their []D values were measured to determine their configuration. Almost all of the daidzein in the medium were metabolized within 48 hrs' incubation with 70% conversion rate to equol by the two strains. Dihydrodaidzein DHD ; which is produced only by NiuO16, was observed in the 12hrs culture broth. Also both of the R- and S-DHD were detected in the broth, only S-equol was formed from DHD by the strain Julong 732, indicating that the biotransformation of DHD is highly stereospecific in this strain. Plasma level of 261110 ng and mean AUC 0-32 h ; of 29571372 ng h ml for genistein has been reported after a single dose of 2 soy extract capsules 22.57 mg genistin ; 13. Plasma levels of genistein were similar after 1 standard dose and at the end of 30 days of daily - intake of 100 mg of soy isoflavones in a recent study21. Thus multiple dosing did not seem to increase plasma levels. Very low levels of plasma genistein 16 ng ml ; were detected in baseline samples in 3 volunteers. Since volunteers on soy foods were not included in this study these could have been generated from other common food items which may have very low concentrations of genistein such as peas and urad. However, all volunteers were instructed not to take soy products 1 month prior to the study and received soy free meals and breakfast on the day of study. The isoflavones are hydrolysed in intestinal enterocytes by glycosidases before absorption. In the liver, in the presence of glucuronic and sulphuric transferases, the conjugated glucuronides and sulphates are formed. These, being more polar circulate in blood to reach various target organs and are excreted in kidney and in the bile. The conjugated isoflavones in the biliary excretion reach the lower intestines. Here these are acted upon by the intestinal microflora and are reabsorbed. This leads to the secondary peak at 6 to h14, 19, 20. The structure of soy isoflavones is similar to the natural synthetic female sex hormone 17 -estradiol and ethinyl estradiol which also undergoes enterohepatic recirculation22. We have earlier reported the secondary plasma peak of both ethinyl estradiol and of norethisterone, in Indian women with low dose oral contraceptives23, 24. The extent to which this occurs with soy isoflavones is likely to be variable. Such variations are also probably dependent on the life long dietary habits rather than short term changes as some authors have reported that there was no effect of short term dietary change on pharmacokinetics of isoflavones25 and ketorolac.

Purified daidzein, genistein, glycitein, 0.9 mmol isoflavone kg diet ; , or group B soyasaponins 2.2 mmol kg diet ; were fed to female golden Syrian hamsters 1112 wk of age, 10 hamsters per group ; for 4 wk in diet containing 37% of energy as fat 62% from coconut oil ; , 25% casein, and 0.1% cholesterol. Hamsters fed glycitein or group B saponins had significantly lower plasma total cholesterol by 16 19% ; and LDL cholesterol by 2738% ; compared with hamsters fed casein P 0.05 ; . Daidzein and genistein's effects on these lipids did not differ from the effects of either casein or glycitein, but group B saponins lowered LDL cholesterol to a greater extent than did daidzein or genistein. Plasma triglycerides were also significantly less in hamsters fed daidzein and group B saponins compared with those fed casein, with the effects of glycitein and genistein not significantly different from any of the other treatments. There were no differences among treatments in plasma HDL cholesterol level. The percentage of urinary recovery of each isoflavonne was glydaidzein genistein 16: 2: 1 ; . The excretion of citein fecal bile acids and neutral steroids were significantly greater in the saponin group compared with the casein group. These data suggest that glycitein's greater cholesterol-lowering effect was due to greater bioavailability as reflected in urinary excretion of glycitein compared with the other isoflavones. Group B soyasaponins seemed to lower plasma total and LDL cholesterol levels by a mechanism involving greater excretion of fecal bile acids and neutral steroids compared with casein. Thus, both soy isoflavones and saponins in nutritionally relevant concentrations contribute to the cholesterol-lowering effects of soy foods and soy protein ingredients and might be useful as purified supplements for cholesterol-lowering as well.

Clinical Summary: Are nonhormonal therapies effective for hot flashes? To assess the efficacy and adverse events of nonhormonal therapies for hot flashes, authors conducted searches of MEDLINE, PsycINFO, Cocharane Database, MANTIS, and AMED for articles up to October 2005 to identify English-language, randomized, double-blind, placebo-controlled trials of nonhormonal therapies. 4249 abstracts were identified. Trials including women with breast cancer were also included. 43 met criteria for review and were considered for meta-analysis. The number of hot flashes per day decreased compared to placebo in the meta-analysis of 7 comparisons of SSRI SNRIs mean difference, -1.13 fewer per day ; , 4 trials of clonidine -0.95 fewer per day at 4 weeks, -1.63 at 8 weeks ; , and 2 trials of gabapentin -2.05 fewer per day ; . The frequency in hot flashes was not reduced for the meta-analysis of red clover and results were mixed for soy isoflavone. Although this provides evidence of efficacy of SSRIs SNRIs, clonidine, and gabapentin in reducing hot flash frequency, this is less than what has been reported for estrogen 2.53 less day ; and many studies were small with only short-term follow-up and methodological flaws. More well-designed, larger trials with long-term follow-up are needed.
