OBJECTIVE: To determine signalment, history, and clinical, necropsy, and microbiologic findings in dairy cows with hemorrhagic bowel syndrome. DESIGN: Retrospective study. ANIMALS: 22 adult dairy cows from a single farm in Colorado. PROCEDURE: Medical records were reviewed for information on signalment, medical and reproductive history, the owner's chief complaints, results of physical examinations and ancillary diagnostic tests, treatment and response to treatment, results of microbiologic testing, and, if applicable, postmortem findings. RESULTS: Common clinical signs were acute signs of profound depression, decreased milk production, tachycardia, ruminal stasis, abdominal distention, and dark clotted blood in the feces. Rectal examination revealed distended loops of small intestine in 7 of cows. Transabdominal ultrasonography revealed small intestinal ileus and distention in 12 of cows and homogeneous echogenic intraluminal material compatible with intraluminal hemorrhage and clot formation in 4. Seven of 8 cows treated medically died; 9 of 13 cows that underwent surgery died or were euthanatized. Clostridium perfringens was isolated from fecal samples from 17 of 20 cows. The most common morphologic diagnosis at necropsy was severe necrohemorrhagic enteritis or jejunitis with intraluminal hemorrhage or blood clots. The most prominent histologic finding was severe, segmental submucosal hemorrhage and edema of the small intestine. CONCLUSIONS AND CLINICAL RELEVANCE: Results confirm that in adult cattle, hemorrhagic bowel syndrome is a sporadic acute intestinal disorder characterized and accolate.

Update on the Therapeutic Duplication Of Atypical Anti-Psychotics Introduction The Mississippi Drug Utilization Review DUR ; Board approved a criterion recommendation and prescriber letter for an intervention regarding the therapeutic duplication of atypical anti-psychotic medications. Methodology Paid claims data are forwarded from ACS to Health Information Designs HID ; for review and evaluation. The DUR Board, Division of Medicaid DOM ; , and HID developed the criterion for this evaluation. In order for a claim exception to occur, a beneficiary must receive at least 2 of the atypical anti-psychotic medications. These medications include ZyprexaTM, SeroquelTM, AbilifyTM, RisperdalTM, and GeodonTM. In June 2003, the existing criterion was amended to therapeutic duplications that occurred for 90 days or longer. For this update, the time span used was January 2003 through December 2003. Claims data were evaluated against the criterion and cases were identified for review by a HID clinical pharmacist. Approved educational intervention letters with attached response forms were mailed to prescribers for identified recipients. The response form asks the prescriber to indicate any action taken in response to the intervention letter. Response forms were returned to HID for review and evaluation. Results A total of 392 profiles were reviewed from 14, 735 possible violations of these criteria. From the 392 profiles selected, 186 beneficiaries were identified who appeared to be available for intervention. Specifically 138 beneficiaries were identified for criterion 454 and 48 for criterion 1431. 92 profiles deleted for either generic prescriber identification number or other quality assurance reasons. After profiles were reviewed, 94 intervention letters were mailed. 25 responses have been received equaling a 27% response rate. Table 1 summarizes the prescriber responses and accolate.
